Beginning now, members can enroll in a healthcare model that removes traditional insurance barriers, offers unlimited access to primary care, and provides predictable costs and support for large, unexpected medical expenses beginning March 1st.

The Health Hub Plus ($190) Monthly Membership Includes:
  • Unlimited in‑office and virtual visits
  • Same‑day/next‑day scheduling
  • Secure messaging with your clinician 
  • Preventative care, chronic condition management & acute care
  • Medications included (from our formulary of 1,200 medications)
  • Many in‑office procedures are included at no additional charge
  • Discounted imaging 
  • Labs included
  • Access to additional nationwide specialists, including Mayo Clinic, MD Anderson, and more
  • Low visit copays for specialists, OBGYN, Chiropractic, Physical Therapy or Counselling Services
  • Diabetic management program (free glucometer and test strips)
  • $1,000 Visa credit for emergency medical or vet bills, with 0% interest for 12 months
  • 50% reimbursement of out-of-pocket cost, up to $1,250 per episode
  • Annual hearing and vision exams
